Cancellation Policy
By scheduling an appointment with MED LAB XPRESS, the client agrees that a $25 fee applies for canceled or missed appointments. (1) no call, no show is allowed; a second will result in the client no longer being eligible for services, and the client must contact their DER or employer to arrange services elsewhere. Fingerprinting and I-9 services carry a $10 administrative fee, and drug testing carries a $10 administrative fee, all due upon arrival time. All payments, including administrative fees, are non-refundable. By submitting payment, the client authorizes MED LAB XPRESS to charge the payment method and agrees not to dispute charges for services, fees, or missed appointments. Submission of payment confirms the client has read and agreed to this policy.
